HPA axis dysfunction, thyroid abnormalities, and hormonal dysregulation in ME/CFS.
What We Know
- HPA axis hypoactivation has been observed in some ME/CFS studies, with reduced cortisol awakening response. This is opposite to the pattern seen in depression, suggesting a biologically distinct mechanism.
What We Think
- Cortisol rhythm studies and adrenal imaging are being used alongside immune profiling to understand neuroendocrine-immune interactions.
What We Don't Know
- Endocrine findings are inconsistent across ME/CFS studies. The clinical significance of mild HPA hypoactivation is unclear. Thyroid abnormalities in ME/CFS are debated.
